Description Reliable Home Care Providers, Inc. is currently seeking a licensed Physical Therapist to join our growing team in a part-time, field-based capacity. This opportunity is well-suited for a clinician looking to build their patient load gradually while maintaining flexibility over their own schedule. The position involves providing one-on-one, skilled therapy in the comfort of the patient’s home. Primary Responsibilities: Conduct in-home physical therapy evaluations and develop individualized treatment plans Deliver skilled physical therapy services aimed at improving mobility, strength, balance, and safety Educate patients and caregivers on home exercise programs and injury prevention Monitor patient progress and adjust care plans accordingly Coordinate with physicians, nurses, and care managers to ensure seamless care Complete accurate, timely clinical documentation in accordance with state and federal regulations Requirements Preferred Candidate: Licensed Physical Therapist in the State of Illinois (required) Minimum of 1 year of experience in home health or adult rehab setting preferred Working knowledge of electronic documentation systems (e.g., Axxess, Kinnser/WellSky, etc.) Strong time management, organizational, and interpersonal skills Must be self-motivated, professional, and reliable Valid driver’s license, reliable transportation, and current auto insurance are required Why Join Our Team: Flexible scheduling that accommodates your availability Support to build and expand your patient base over time Competitive per-visit compensation Mileage reimbursement and bi-weekly pay schedule Administrative and clinical team support readily available Coverage Area: We are currently prioritizing candidates who can serve the Northside Chicago neighborhoods, including but not limited to: Uptown, Andersonville, Rogers Park, Edgewater, Lincoln Square, and nearby communities.
