ABOUT THE ROLE:
As a Physical Therapist in Home Health, you will play a critical role in delivering personalized rehabilitation services to patients within their own homes, promoting recovery and enhancing their quality of life. You will assess patients’ physical conditions, develop tailored treatment plans, and implement therapeutic interventions to improve mobility, reduce pain, and prevent disability. This position requires collaboration with interdisciplinary healthcare teams to ensure comprehensive care and effective communication with patients and their families to educate and motivate them throughout the recovery process.
