Description About the Role: Sierra Home Health is seeking a passionate and patient-focused Physical Therapist to join our dedicated team in Truth or Consequences, NM. You'll work independently with the support of a tight-knit clinical team, delivering personalized care in a community that values relationship-driven healthcare. Key Responsibilities: Deliver physician-ordered physical therapy services in patients' homes Evaluate patient function and create individualized care plans Conduct initial assessments, reassessments, and plan of care revisions every 30 days Supervise Physical Therapy Assistants and Home Health Aides as needed Educate patients and caregivers on exercises and home care routines Collaborate with the care team and physicians on treatment adjustments Prepare timely and accurate clinical documentation, including OASIS Contribute to quality initiatives, in-services, and patient care conferences Requirements What We’re Looking For: Current licensure as a Physical Therapist in New Mexico Graduate of an accredited Physical Therapy program Strong assessment and care planning skills Ability to work independently and manage a home-based caseload Excellent communication with patients, families, and team members Valid driver’s license and reliable transportation Prior home health experience preferred, but new grads are welcome!
