Description Klarus Home Health is seeking a PRN / Per Diem Licensed Physical Therapist to join our team to providing PT services including assessment, treatment planning, implementation, and evaluation for our patients currently enrolled in our home health program in the Dallas/Dallas South region and surrounding areas. Requirements Must be willing to do a Saturday rotation approximately once every 4-6 weeks Evaluate patients' physical and functional abilities to develop individualized care plans. Plan, implement, and modify physical therapy treatment programs based on patients' medical referrals and assessments. Provide hands-on therapy, exercises, and techniques to help patients improve mobility, relieve pain, and restore function. Educate patients and their families on therapeutic exercises, proper body mechanics, and injury prevention techniques. Collaborate with interdisciplinary healthcare teams to ensure coordinated and holistic patient care. Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans. Participate in ongoing professional development activities to stay abreast of current trends and best practices in physical therapy. Experience: Graduate of an accredited School of Physical Therapy. Current State license as a Physical Therapist, or a multi-state license issued by a state member of the National Physical Therapy Compact (PT Compact), if practicing in a state member of the PT Compact. Minimum of two years of experience as a Physical Therapist in a clinical care setting, with experience in Home Health, geriatrics, or medical-surgical preferred. Excellent interpersonal and communication skills. Strong nursing skills as defined by generally accepted standards of practice. Reliable transportation with valid and current auto liability insurance. Ability to work flexible hours and travel locally as needed. Ability to lift up to 50 lbs. and perform physical tasks associated with patient care.
