JOB OVERVIEW

We are seeking a dedicated and compassionate Physical Therapist or Physical Therapist Assistant to join our dynamic healthcare team.
The ideal candidate will work closely with licensed physical therapists to implement treatment plans, assist patients in their rehabilitation journey, and contribute to their overall recovery. This role requires a strong understanding of patient care, anatomy, and physiotherapy practices, as well as the ability to work effectively in home health environments.

SKILLS

  • Strong knowledge of anatomy and physiology relevant to physical therapy practices.
  • Familiarity with medical terminology related to patient care.
  • Experience in home & community care settings is preferred.
  • Background in inpatient hospital experience or outpatient clinic experience is a plus.
  • Ability to develop effective care plans tailored to individual patient needs.
  • Excellent communication skills for interacting with patients, families, and healthcare professionals.
  • Proficiency in using EMR systems for documentation purposes.
  • Understanding of various therapeutic techniques including physiotherapy methods.

Responsibilities
  • Provide direct patient care, ensuring comfort and safety during therapy sessions.
  • Monitor patient progress and report any changes or concerns to the supervising therapist.
  • Participate in discharge planning and collaborate with the healthcare team to ensure continuity of care.
  • Maintain accurate documentation of patient treatments using EMR systems.
  • Educate patients and their families on treatment plans and home exercises.
  • Support patients with a variety of conditions including orthopedics, geriatrics, pediatrics, and acute care needs.
  • Utilize techniques such as trigger point therapy and other physiotherapy modalities as directed.
