Job Description Summary: Competently interpret the patient's history and baseline level of function and then to complete an assessment appropriate to the patient's area(s) of deficits. Based on problem(s) identified, the physical therapist establishes and carries out a plan of care to include patient treatment and reassessment, patient/family instruction, discharge planning, documentation in the medical record and providers and other medical staff. Oversees and supervises provision of care by physical therapist assistants. Responsibilities And Duties: Direct Patient Care 90-100% Evaluates and treats patients: After provider referral, evaluates patient physical abilities and functional status, develops and implements plan of care based on evaluation findings and appropriate treatment techniques and initiates patient treatment. Carries out plan of care with reassessment and changes to plan of care as appropriate. Responsible for maintaining/meeting the productivity expectations of their department. Oversees and supervises provision of care by physical therapist assistants. Plans and helps coordinate discharge plan in collaboration with the multidisciplinary team: Instructs patient and care giver/family. Recommends DME required for discharge. Provides information to providers, nurses, case managers, social workers, and other team members. Involved in the multidisciplinary patient plan of care and discharge readiness plan. Documents evaluation, plan of care, progress toward goals, discharge plan/continuity of care for multidisciplinary team, and other involved medical and insurance parties. Indirect Patient Care 1-10% Department Functions: Supervises patient assignments and duties of the PTAs and rehabilitation tech(s). Is involved in department quality improvement through data collection and participation in team and staff meetings. Assumes duties delegated by management in the absence of department management. Education/Mentoring: Takes an active role in orienting new staff members. Serves as clinical instructor for affiliating PT and PTA students. Serves as an advocate for discipline on the multidisciplinary team. Represents department at problem-solving and committee meetings.
