Job Details Job Location: Topeka, KS 66614 Position Type: Part Time Job Shift: Day Join our interdisciplinary team at Midland Care, a proud provider of PACE services, dedicated to enhancing the lives of older adults by supporting their independence and quality of life. Midland Care’s Program of All-inclusive Care for the Elderly (PACE) is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement stay in their own homes and communities. Employment Type: Part Time | 24/week between M-F 8am-5pm Pay Rate: $40+/hour (pay is based on qualifications and experience) Location: Topeka, KS We are seeking a dedicated and compassionate Physical Therapist to join our Program of All-Inclusive Care for the Elderly (PACE) team. In this part-time role, you will work closely with an interdisciplinary team to provide rehabilitative services to frail elderly participants, helping them maintain independence and improve quality of life. Key Responsibilities: Perform assessments and develop individualized treatment plans Collaborate with primary care providers, occupational therapists, and other team members Document progress and adjust plans as needed Educate participants and caregivers on mobility, safety, and exercises What We Provide for You: Free Employee Assistance Program (EAP) for employees and dependents 403(b) ERISA Retirement Plan, match provided On-site education center to provide professional development and CEU credits Qualifications Required: Graduate of an accredited entry-level program in physical therapy approved by the American Physical Therapy Association, The Committee on Allied Health Education and Accreditation of the American Medical Association, the council on Medical Education of the American Medical Association and the American Physical Therapy Association, or other equivalent organizations approved by the Secretary, Bachelor’s degree in physical therapy. Current unrestricted Kansas State license to practice as a physical therapist. Must possess a valid Kansas Driver’s License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization’s insurability standard as defined by Midland policy. Availability of reliable transportation is required for this position. One year of professional experience required, minimum one year experience working with the frail and elderly population, home care or geriatric experience.
