Physical Therapist (PT) or Physical Therapist Assistant (PTA) – Laurens, IA Blue Stone Therapy is seeking a Physical Therapist or Physical Therapist Assistant to provide rehabilitation services to adult and geriatric patients at Laurens Care Center in Laurens, IA. This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Physical Therapist is responsible for patient assessments, care plan development, and implementation of treatment programs designed to improve gait, balance, mobility, strength, range of motion, pain management, and overall functional independence. [Optional line if multi-site:] This position will serve multiple partner communities, including Fonda Specialty Care and Accura HealthCare of Pomeroy, with travel between locations as needed. What You’ll Do – These will be different based on the position • Provide skilled physical therapy services to adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting • Perform patient evaluations and develop plans of care (PT) or implement treatment programs under the direction of a supervising Physical Therapist (PTA) • Deliver therapy focused on gait training, balance, mobility, strength, range of motion, endurance, and pain management • Monitor patient response to treatment and adjust interventions in collaboration with the therapy team • Maintain appropriate patient schedules per established guidelines • Complete timely and accurate documentation to support quality care and reimbursement requirements • Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes Requirements What You Bring – These will be different based on the position • Graduate of an accredited Physical Therapy or Physical Therapy Assistant program • Current PT or PTA license (or eligibility for licensure) in the applicable state • Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred • Strong clinical reasoning, communication, and organizational skills • Commitment to patient-centered care and professional standards Why Blue Stone Therapy?
