Description Physical Therapist / Physical Therapist Assistant (PRN) – Continuing Care Retirement Community HTS is currently seeking dedicated and compassionate Physical Therapists and Physical Therapist Assistants to join our therapy team on a PRN basis at our partner continuing care retirement community located in Walnut Creek, Ohio (Holmes County). Key Responsibilities: - Provide high-quality physical therapy services to residents in a continuing care retirement community. - Assist with managing increases in caseload, scheduled time off, and weekend needs on a PRN basis. - Maintain accurate documentation of resident progress and treatment plans. - Collaborate with the interdisciplinary team to develop and implement individualized care plans. - Ensure compliance with all regulatory and safety standards. - Adapt therapy sessions to meet the unique needs of each resident, promoting mobility, independence, and overall well-being.
