Description
Assess, plan, organize, and participate in rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury.  As a Physical Therapist, you will: * Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention. * Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients. * Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.  * Applies concepts of infection control/universal precautions in performing patient care * Assure continuity of quality patient care delivered with appropriate documentation * Provide effective communication to patient/family, team members and other health care professionals * Promote agency philosophy and administrative policies * Maintain and complete all documentation within company stated guidelines.  Requirements * Successful completion of accredited physical therapy program and state licensure exam. * Valid Physical Therapy license * 1 year of home health experience preferred * CPR certification * Valid Driver’s license * Valid Auto Insurance * HomeCare HomeBase experience preferred Physical Demands * Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods. * Must be able to lift, carry, pull, or push up to 100 lbs..
Responsibilities
