We are seeking a compassionate and skilled PRN Physical Therapist to provide high-quality care to patients in a home health setting. In this role, you will assess, plan, and implement individualized therapy programs designed to improve mobility, relieve pain, restore function, and enhance overall quality of life. This is an ideal opportunity for a therapist who values autonomy, flexibility, and meaningful patient relationships. ---------------------------------------- Key Responsibilities * Perform and document comprehensive initial evaluations and ongoing assessments * Develop and implement individualized treatment plans based on patient needs and goals * Provide therapeutic interventions to improve strength, mobility, and functional independence * Monitor patient progress and adjust treatment plans for optimal outcomes * Ensure timely, accurate, and compliant clinical documentation * Educate patients and caregivers on treatment plans and home exercise programs * Collaborate effectively with interdisciplinary team members and physicians * Follow infection control and safety protocols at all times ---------------------------------------- Qualifications * Graduate of an accredited Physical Therapy program * Current, active Physical Therapist license * Minimum 1 year of experience (home health preferred) * Current CPR certification * Valid driver’s license and reliable transportation with auto insurance * Experience with HomeCare HomeBase (HCHB) preferred ---------------------------------------- Physical Requirements * Ability to stand, walk, bend, kneel, and squat for extended periods * Ability to lift, carry, push, or pull up to 100 lbs
