Job Summary The Physical Therapist evaluates, plans, and administers therapeutic procedures in physical medicine and rehabilitation as prescribed by a referring physician. This role establishes individualized physical therapy goals, provides treatments, and regularly reassesses patient progress, documenting all findings accurately. Essential Functions * Conducts thorough physical therapy evaluations to assess patient needs and develops individualized treatment plans. * Administers safe and effective therapy treatments, ensuring all interventions are appropriately documented. * Reassesses and adjusts treatment plans within appropriate timeframes to align with patient progress and changing needs. * Provides clear, concise, and accurate documentation of evaluations, treatment plans, progress notes, and billing information. * Educates patients and their families on treatment plans, therapeutic exercises, and post-treatment care to support long-term recovery. * Operates therapy equipment safely and maintains a clean, organized, and safe environment for patients and staff. * Collaborates with referring physicians, peers, and interdisciplinary team members to ensure continuity and quality of patient care. * Demonstrates population-appropriate competencies for all relevant patient demographics, tailoring care to individual needs. * Maintains compliance with facility policies, including safety, infection control, and risk management procedures. * Adapts to changes professionally and effectively, following the chain of command to address and resolve issues impacting patient care. * Performs other duties as assigned. * Maintains regular and reliable attendance. * Complies with all policies and standards. Qualifications * Doctorate in Physical Therapy (DPT) required * 0-2 years of experience in a physical therapy setting required Knowledge, Skills and Abilities * Strong knowledge of physical therapy principles, therapeutic techniques, and safety standards. * Ability to perform comprehensive evaluations and develop effective treatment plans. * Proficiency in using therapy equipment and technology for documentation and billing. * Excellent communication and interpersonal skills to collaborate with patients, families, and interdisciplinary teams. * Strong organizational and time management skills to prioritize tasks and maximize productivity. * Commitment to professional development and adherence to ethical and legal standards. Licenses and Certifications * PT - Physical Therapist license in the state of employment required * BCLS - Basic Life Support within 30 days of hire required
