Our Rehab services include inpatient, outpatient, and swing bed (subacute) rehabilitation, offering physical therapists the opportunity to make a meaningful impact across a variety of care settings. Position Summary: We are seeking a compassionate and flexible Physical Therapist (PT) to join our team on a PRN (as needed) basis. The PT will evaluate, plan, and implement individualized treatment programs for patients across multiple care settings, working closely with physicians, nurses, and other therapy professionals to promote recovery, independence, and wellness. Responsibilities: Provide physical therapy services for inpatient, outpatient, and swing bed (subacute) patients. Complete assessments and develop individualized treatment plans. Deliver evidence-based therapy interventions to improve mobility, strength, and function. Educate patients and families on therapy goals, exercises, and discharge planning. Collaborate with an interdisciplinary team to support patient progress and outcomes. Document care accurately and timely in the medical record. Qualifications: Current licensure as a Physical Therapist (PT) in the state of Michigan. Graduate of an accredited Physical Therapy program. Prior experience in inpatient, outpatient, or swing bed settings preferred, but not required. Strong communication, organization, and patient-centered care skills.
