The Physical Therapist PRN at our Skilled Nursing Facility (SNF) in San Augustine, TX, plays a critical role in delivering high-quality rehabilitative care to patients recovering from illness, injury, or surgery. This position focuses on assessing patients' physical conditions, developing personalized treatment plans, and implementing therapeutic interventions to improve mobility, reduce pain, and enhance overall functional independence. The therapist will collaborate closely with interdisciplinary teams, including physicians, nurses, and occupational therapists, to ensure comprehensive patient care and optimal recovery outcomes. As a PRN (pro re nata) role, the therapist will provide flexible, as-needed services, adapting to fluctuating patient census and facility demands. Ultimately, this position contributes significantly to improving patients' quality of life and facilitating their safe return to home or community settings. As a Physical Therapist, you will: * Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention. * Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients. * Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.  * Applies concepts of infection control/universal precautions in performing patient care * Assure continuity of quality patient care delivered with appropriate documentation * Provide effective communication to patient/family, team members and other health care professionals * Promote agency philosophy and administrative policies * Maintain and complete all documentation within company stated guidelines.  Requirements * Successful completion of accredited physical therapy program and state licensure exam. * Valid Physical Therapy license * 1 year of home health experience preferred * CPR certification * Valid Driver’s license * Valid Auto Insurance * HomeCare HomeBase experience preferred Physical Demands * Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods. * Must be able to lift, carry, pull, or push up to 100 lbs..
