QUALIFICATIONS         Education * B.S., M.S., D.P.T. or Post-Baccalaureate Certificate in Physical Therapy * Graduate of an approved school of Physical Therapy or graduate of accredited college or university with a certificate in Physical Therapy         Experience/Skills * Three years previous Physical Therapy experience * Demonstrates knowledge of physical medicine, physical therapy modalities, anatomy, and physiology. * Reads and communicates effectively in English * Possesses strong written and verbal skills * Exhibits professionally appropriate telephone communication skills         Required Licenses/Certifications * Physical Therapist licensed in the State of Indiana * CPR         Working Conditions * Clean, well-lit working environment * Physical demands as related to movement, lifting, environment, vision, hearing, and repetitive motion * Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions of the position without compromising client care     ROUTINE RESPONSIBILITIES         Behavioral Expectations * Consistently complies with established Behavioral Expectations         Therapist Duties * Accepts responsibility for evaluation, planning, directing, and administering physical therapy treatment to clients/patients as prescribed by a licensed provider according to the Indiana standards of physical therapy practice * Supervises and directs treatment by Physical Therapy Assistants (PTAs) and supervises clerical staff * Participates in operational aspects of the department including assisting with budget input, expense management, assisting in clerical duties, and cleaning of department as needed * Participates in departmental performance improvement and quality initiatives         Communication * Communicates progress and provides feedback effectively with patients and families as needed in regard to their physical therapy care * Communicates effectively with physicians and other prescribing providers regarding any concerns or changes in treatment * Professionally communicates with nursing and other therapy staff regarding patient care as needed         Documentation * Completes patient documentation in a clear appropriately detailed, and timely manner that meets current standards and policies * Documents patient charges accurately and with attention to time constraints         Department-Specific Goals * Maintains hours worked within budget * Promotes excellence in patient satisfaction scores with scripting, capturing of patient emails, and discussions with patients about the overall importance and protocol of Press Ganey surveys Varies as needed.
