Description PRN position available for primarily day shift and some weekends. We are seeking a Physical Therapist to join our Therapy Services team. The PT shall screen, evaluate, re-evaluate, discharge plan and make revisions in any established plan of care as supervised by the physician Evaluate residents per MD order and establish appropriate Treatment Plans and Goals that meet the specific needs of the individual. Re-assess patients and caseload as needed in order to determine need for continued services or discontinuation of services based on patient progress. Carry out treatment plan as indicated. Supervise treatment implementation carried out by Licensed Physical Therapy Assistants and determine appropriateness, as well as modify treatment plan as indicated. Provide thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment. Educate staff, family, and care givers as indicated. Identify needs for program development and implement programs according to company policies and procedures. Identify equipment needs of Physical Therapy Dept. and order equipment according to company and facility policy. Complete all required documentation according to guidelines specified in Documentation Manual. Coordinate in-services for facility staff as need is determined, to ensure safe and effective carry over of Maintenance/Restorative Programs and overall call of the residents. Requirements Must have a degree in Physical Therapy from an accredited program Active state license required Possess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines. Ability to supervise PTAs and PT Techs Westminster Canterbury of Lynchburg is committed to best practices for infection control; therefore, we will require all team members to receive the influenza vaccine and recommend COVID-19 vaccines.
Responsibilities
The Physical Therapist will evaluate residents, establish treatment plans, and supervise the implementation of care by assistants. They are also responsible for discharge planning, staff education, and maintaining accurate clinical documentation.
