New clinic opening in late June Full Time hours: Monday-Friday 8am to 5:30pm JOB SUMMARY The Physical Therapist is responsible for evaluating and treating patients with musculoskeletal injuries, movement dysfunction, and functional impairments. This role focuses on restoring mobility, improving physical function, reducing pain, and preventing future injury through evidence-based therapeutic interventions. The Physical Therapist works collaboratively with primary care providers, nurses, behavioral health clinicians, and other health center staff to deliver coordinated, patient-centered care in an employer-sponsored health center environment. This position emphasizes early intervention, return-to-function, injury prevention, and patient education to support employee health, productivity, and long-term wellness. ESSENTIAL DUTIES AND RESPONSIBILITIES Patient Evaluation and Assessment Perform comprehensive physical therapy evaluations including review of medical history, functional assessment, and musculoskeletal examination Identify impairments in mobility, strength, balance, posture, and movement patterns Develop individualized treatment plans based on clinical findings and patient goals Establish measurable functional outcomes and progress benchmarks Treatment and Rehabilitation Provide evidence-based therapeutic interventions including o Therapeutic exercise o Manual therapy techniques o Neuromuscular re-education o Functional training o Balance and gait training o Postural correction Implement individualized rehabilitation programs designed to restore function and reduce injury recurrence Monitor patient progress and modify treatment plans as clinically appropriate Injury Prevention and Wellness Educate patients on injury prevention, ergonomics, and proper body mechanics Provide workplace ergonomic assessments and recommendations when appropriate Support wellness initiatives designed to reduce musculoskeletal injury risk Care Coordination Collaborate with primary care providers and other health center clinicians to coordinate patient care Participate in multidisciplinary care planning and case discussions Refer patients to appropriate specialists when additional evaluation or treatment is required Patient Education Educate patients on treatment goals, exercise programs, and self-management strategies Provide guidance on safe return to activity, work duties, and recreational activities Encourage patient engagement in long-term musculoskeletal health Documentation and Compliance Maintain timely, accurate, and complete documentation in the electronic medical record (EMR) Ensure treatment plans and progress notes meet regulatory and professional standards Adhere to all health center policies, clinical protocols, and professional practice guidelines REQUIRED KNOWLEDGE, SKILLS, & ABILITIES Required Qualifications · Doctor of Physical Therapy (DPT) from an accredited physical therapy program · Current and unrestricted license to practice as a Physical Therapist in the state of employment Preferred Qualifications · Orthopedic Clinical Specialist (OCS) · Certification in manual therapy or advanced musculoskeletal rehabilitation · Basic Life Support (BLS) certification Experience · Two or more years of clinical physical therapy experience preferred · Experience in outpatient orthopedic or sports rehabilitation settings strongly preferred · Experience working in integrated or employer-sponsored health clinics is a plus Proactive MD is firmly committed to creating a diverse workplace and is proud to provide equal employment opportunities to all employees and applicants for employment without regard to race, color, religion, sex, gender identity and/or expression, sexual orientation, ethnicity, national origin, age, disability, genetics, marital status, amnesty status, or veteran status applicable to state and federal laws.
