SUMMARY

We are seeking an in office Physical TherapistProgram Coordinator to join our team at OneWell Health Care. In this role, you will work closely with individuals with IDD, developing and implementing personalized physical therapy plans that enhance mobility, strength, and overall quality of life. Your expertise will play a vital role in helping our clients achieve their health goals.

REQUIREMENTS

• Licensed Physical Therapist in the state of practice.
• Strong knowledge of anatomy, physiology, and physical therapy techniques.
• Experience in developing care plans tailored to individual needs.
• Ability to work collaboratively within a team-oriented environment.
• Excellent communication skills for interacting with patients, families, and healthcare professionals.

• Evaluate patients’ physical abilities and develop individualized treatment plans.
• Implement therapeutic exercises and techniques to improve mobility and strength.
• Collaborate with interdisciplinary teams to ensure comprehensive care for clients.
• Educate patients and families on treatment plans and home exercise programs.
• Monitor patient progress and adjust treatment plans as necessary.
• Maintain accurate documentation of patient care activities and outcomes.
• Provide support for discharge planning to ensure continuity of care.
