As a Home Health Physical Therapist, you'll develop individualized treatment plans, provide hands-on care, and build meaningful relationships with patients and families while enjoying the flexibility and autonomy that comes with community-based care. WHAT YOU'LL DO * Evaluate patients and develop personalized treatment plans. * Improve mobility, strength, balance, and overall function. * Educate patients and caregivers on exercises, safety, and disease management. * Monitor progress and adjust treatment plans to maximize outcomes. * Collaborate with physicians, nurses, and interdisciplinary team members. * Complete timely and accurate clinical documentation. QUALIFICATIONS * Graduate of an accredited Physical Therapy program. * Current Physical Therapist license. * CPR certification. * Valid driver's license and auto insurance. * Home Health experience preferred. * HomeCare HomeBase experience preferred.
