We are currently looking for a compassionate, organized, and team-oriented Home Health Physical Therapist (PT) - Level 3 to serve patients throughout the San Mateo Peninsula. Must be proficient in OASIS and case management. Experience: 4+ years of Home Health experience. Must be an expert in complex case management. Clinical specialization is a plus. Coverage Area: Regional Floater. Must be willing to cover the wider service area based on highest priority patient needs. Schedule: High Availability. Willingness to work weekends or urgent coverage shifts as needed. PTA : Substantial PTA supervision Ability to travel within San Francisco and surrounding areas Caring, compassionate personality with strong communication skills Job Type: Full-Time / Per Diem Schedule: Flexible. Monday – Friday standard, with rotational weekend coverage (1 weekend per month/quarter) required. Annual Full-Time Expectation: $170,000 – $190,000 per year Pay: $81.00 – $91.00 per hour This is NOT a contracted position
