Description Facility: Sylva Valley Health & Rehab (Skilled Nursing Facility) Job Type: Full-Time (Part-Time/PRN considered) About the role Sylva Valley Health & Rehab is hiring a Physical Therapist (PT) and/or Physical Therapist Assistant (PTA) to provide high-quality rehab services in a skilled nursing setting. This role is ideal for clinicians who value patient-centered care, strong clinical outcomes, and teamwork with nursing and the interdisciplinary team. What we offer Competitive pay (based on experience) Flexible schedule options (discussed during interview) Supportive team culture and collaborative rehab environment Vacation time + Sick time (per eligibility) Key responsibilities (PT) Evaluate residents and develop individualized plan of care Provide PT treatment to improve mobility, strength, balance, gait, and safety Document thoroughly to support skilled coverage and clinical progress Coordinate with OT, SLP, nursing, and providers regarding changes in condition Educate residents and families on exercises, safety, and discharge planning Participate in care conferences/IDT as needed Key responsibilities (PTA) Provide treatment interventions under the supervision of a PT Document treatments and resident response/progress Communicate changes in condition to the supervising PT and care team Assist with resident and family education per plan of care Support discharge planning and continuity of care Requirements PT: Current Physical Therapist license + graduate of an accredited PT program PTA: Current Physical Therapist Assistant license + graduate of an accredited PTA program Strong documentation and communication skills SNF/rehab experience preferred (new grads considered) Apply Apply online: tinyurl.com/sylvavalley

Responsibilities

Physical Therapists will evaluate residents, develop individualized plans of care, and provide treatment to enhance mobility, strength, balance, and safety.
