It is our goal to DELIVER excellent post-acute care rehabilitation experiences to every patient entrusted to our care. We need your help to complete our team and together we will DELIVER exceptional care and clinical outcomes. Physical Therapist (PT) - PRN Come Join our Facility Team! Responsibilities * Facilitation of rehabilitation of patients with emotional, physical or mental disabilities * Implementation of occupational therapy care plans * Coordination of care with entire care team * Educating patients and family members * Preparing timely, complete and accurate clinical notes * Communicating effectively with patients, caregivers, healthcare providers, office support and other care team members Qualifications * Believes in a team approach to problem solving and care Freely * Physical Therapist in the State of Texas * Skilled Nursing, Healthcare (Long Term Care) or Veterans Home experience highly preferred * Knowledge of Home Health regulations and general healthcare compliance * Experience with EMR * Truly cares for people and will go above and beyond to provide outstanding patient care * Love flexibility that allows you to do your best work while enjoying great quality of life * Has a passion for therapy and teaching We are an equal opportunity employer and prohibit discrimination/harassment without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.
