Concept Rehab is seeking a licensed Physical Therapist (PT) to provide high-quality rehabilitation services in a post-acute rehabilitation setting. As a PRN Physical Therapist, you'll have the flexibility to pick up shifts that fit your schedule while working with an experienced interdisciplinary team dedicated to patient-centered care. What You'll Be Doing Perform comprehensive physical therapy evaluations. Develop individualized treatment plans based on each patient's goals and functional abilities. Deliver evidence-based physical therapy interventions to improve strength, balance, mobility, endurance, and safety. Monitor patient progress and adjust treatment plans as appropriate. Collaborate with Occupational Therapy, Speech Therapy, nursing, physicians, and caregivers. Complete accurate, timely clinical documentation. Qualifications Doctor of Physical Therapy (DPT) or equivalent from an accredited program. Current Physical Therapist license in the State of Michigan. Strong clinical decision-making and communication skills. Experience in rehabilitation, post-acute care, or geriatric therapy is welcomed but not required. New graduates are encouraged to apply.
