You’ll work under the direction of the Regional Director of Therapy, delivering care based on physician orders and individual plans of care. Key Responsibilities: Assesses patient functional ability and potential. Completes PT evaluation to determine potential benefit from therapy and to establish an effective plan of treatment. Develops and implements medically prescribed PT plan of treatment. Prepares written plan of treatment for physician ‘s signature, including clinical findings, goals, and rehab potential. Assures that all PT orders include specific procedures and modalities to be used with a specific emphasis on amount, frequency, and duration of treatment. Understands and interprets medical information from patient record and rest of interdisciplinary team. Establishes goals and discharge plans. Interprets and adapts physician orders. Coordinates treatment plan with interdisciplinary team. Instructs and supervises PTA in provided services and evaluates performance and effectiveness and progress of services provided at least every 30 days. Instructs and supervises patient/caregiver/family member in aspects of PT to be carried out. Accurately and completely documents each visit in a timely manner. Communicates with physician and interdisciplinary team all appropriate data and changes in patient condition, complications, and additional needs. Orders and /or supervises ordering of specialized durable medical equipment. Attends and participates in case conference and agency in-service programs. Other duties as assigned. Requirements Qualifications: Bachelor’s degree (minimum) in Physical Therapy from an accredited program Licensed Physical Therapist in Missouri (and any other state of practice) National Board for Certification in Physical Therapy Minimum of 2 years of experience preferred Physical Demands: Ability to participate in physical activities for extended periods Moderate lifting and regular bending/standing required Work Environment: Clinical, residential, and home-based care settings Frequent local travel between patient locations Exposure Risk: High
Responsibilities
The Physical Therapist will assess patient functional ability and develop medically prescribed treatment plans to be delivered in residential or home settings. They are responsible for supervising Physical Therapist Assistants, documenting patient progress, and coordinating care with the interdisciplinary team.
