Duties: Provides formal and informal evaluation of functional capabilities of clients on assigned caseload. Reviews results with clients and their families as well as classroom staff when indicated. Develops and implements individual treatment plans and ongoing treatment programs addressing specific goals with measurable outcomes. Schedules clients to meet requirements of education plan. Maintains written documentation of all evaluations, Individual Education Plan (IEP) goals, PT treatment plans, progress notes and communication with doctors and families in a professional, compliant, and timely manner. Participates actively, cooperatively, and constructively as a member of a multidisciplinary team. Provides instruction and guidance to ensure that the clients’ IEP goals are integrated into a comprehensive educational program. Meeting Street believes the dignity of each person is paramount. Licensure as a registered physical therapist by the Massachusetts Department of Health of Health. A minimum of one (1) year related experience; familiarity with a pediatric population preferred. Valid CPR and First Aid training.
