This role involves working closely with clients and their healthcare teams to deliver exceptional therapeutic care in a personalized and professional environment. Position Overview The in-home Physical Therapist will evaluate, develop, and implement personalized treatment plans for clients, focusing on improving mobility, reducing pain, and enhancing quality of life. This role requires excellent interpersonal skills, clinical expertise, and the ability to work with diverse patient populations, including high-profile clients. The ideal candidate is compassionate, detail-oriented, and committed to achieving positive outcomes for clients while adhering to the highest standards of patient care and confidentiality. Key Responsibilities Patient Care: • Conduct comprehensive physical therapy evaluations to assess patient conditions, needs, and goals. • Develop and implement individualized treatment plans, incorporating therapeutic exercises, manual therapy, and modalities as appropriate. • Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes. • Educate patients and their families on exercises, home programs, and injury prevention strategies. Collaboration: • Work closely with physicians, nurses, and other healthcare providers to ensure coordinated and comprehensive care. • Provide progress reports and communicate treatment adjustments to the care team as necessary. Documentation & Compliance: • Maintain accurate, thorough, and timely documentation of evaluations, treatment sessions, and progress notes in compliance with state, federal, and organizational guidelines. • Adhere to all HIPAA regulations to protect patient confidentiality. Client Relations: • Deliver exceptional, patient-centered care while maintaining professionalism and discretion, particularly with high-profile clients. • Address patient concerns promptly and effectively, ensuring a positive therapy experience. Professional Development: • Stay current on physical therapy practices, techniques, and industry trends through continuing education and professional development. • Participate in team meetings, training sessions, and quality improvement initiatives. Required: • Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program. • Current California Physical Therapist License in good standing. • 1+ year of experience as a licensed Physical Therapist (preferred in private duty or concierge settings). • Strong knowledge of therapeutic exercise techniques, manual therapy, and rehabilitation protocols. • Excellent communication and interpersonal skills to build rapport with clients and collaborate with the care team. • Ability to work with a diverse patient population, including high-profile clientele, while maintaining confidentiality and professionalism. • CPR and First Aid certification (or willingness to obtain). Preferred: • Experience in concierge healthcare or private-duty physical therapy. • Familiarity with electronic medical record (EMR) systems. • Certifications in specialized areas such as orthopedics, sports therapy, or manual therapy. Schedule & Compensation • Per Diem • Flexible schedule to accommodate client needs, including occasional evenings or weekends. • Compensation: Per Visit Requirements Required: • Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program. • Current California Physical Therapist License in good standing. • 1+ year of experience as a licensed Physical Therapist (preferred in private duty or concierge settings). • Strong knowledge of therapeutic exercise techniques, manual therapy, and rehabilitation protocols. • Excellent communication and interpersonal skills to build rapport with clients and collaborate with the care team. • Ability to work with a diverse patient population, including high-profile clientele, while maintaining confidentiality and professionalism. • CPR and First Aid certification (or willingness to obtain).
