QUALIFICATIONS

  • Current Florida professional licensure as a Physical Therapy Assistant
  • Must have long term care experience. Knowledge of PDPM, Section GG and LTC program development.
  • Familiar with Net Health and Point Click Care

Responsibilities
  • Provides treatment and documentation for residents assigned to caseload
  • Provides appropriate physical therapy interventions based off of resident evaluation; collaborates cooperatively with other patient team personnel in maintaining standards for professional physical therapy practice and interdisciplinary teamwork.
  • Observes and assesses treatment affect, accomplishment of short- and long-term goals. Recommends change to Physical Therapist, if needed
  • Performs an appropriate screening on each patient as per policy. Performs accurate and timely GG scoring. The population is adult and geriatric residents in an SNF setting.
  • Able to assess resident pain interfering with optimal level of function of participation in rehabilitation; makes appropriate interventions to reduce pain; makes appropriate physician contact for intervention.
  • Follows resident specific plan of care for physical therapy.
  • Other duties as assigned.
