Responsibilities
  • Travel daily to assigned MN service areas to visit clients in their private homes
  • Administer physical therapy treatments, exercises, and manual modalities under PT supervision, ensuring safe use of equipment and effective mobility assistance
  • Track and report client progress, measure vital signs and functional status, and educate on the use of braces, prosthetics, and assistive devices
  • Collaborate with the care team for treatment planning, maintain thorough documentation, uphold HIPAA compliance, and respond appropriately to emergencies
  • Promote a positive agency image, support clients with daily activities, and perform additional duties as needed to enhance client care and service
