QUALIFICATIONS

• Active PA or NP license in [State] with national certification (NCCPA or NP board).
• 1+ year experience in primary care, internal medicine, sleep medicine, or chronic disease management preferred.
• Comfortable with telehealth platforms, EMRs, and remote device troubleshooting.
• Excellent communication and patient engagement skills.

ABOUT THE ROLE

We are seeking a compassionate PA or NP to provide proactive, technology-enabled care through telehealth and RPM. You will review patient data, conduct virtual visits, and collaborate with the care team to manage chronic conditions, prevent complications, and improve patient outcomes.

RESPONSIBILITIES

• Review and interpret RPM data (Sleep data, blood pressure, oxygen levels, weight, glucose, etc.).
• Conduct telehealth visits and chronic care follow-ups.
• Adjust care plans in collaboration with supervising physician.
• Educate patients on device use, self-monitoring, and lifestyle changes.
• Document care in EMR for CMS RPM/CCM billing compliance (99453, 99454, 99457, 99458).
• Coordinate with clinical team to address alerts and support quality improvement.
