Job Purpose  Physician’s Case Manager (PCM) is responsible for monitoring all persons served receiving medication services only. PCM collaborates with the treatment team and Director to best meet the needs of persons served and quality improvement measures of the program. The PCM provides case management services to persons served receiving only medication services.  Qualifications  * PCM must have a minimum Associates degree in the humanities combined with experience/training in a behavioral health program or a 4-year degree in a related field with experience. Core Job Duties  * Collaborate with person served to create a treatment plan with measurable goals and objectives  * Monitor progress toward outcomes specified in the plan * Link person(s)-served with community resources as recommended by the Psychiatrist  * Develop safety plans that involve natural supports and review on going effectiveness of safety plan  * Provide lethal means counseling if needed and ensure restriction of lethal means when high risk identified  * Attend staffing weekly to discuss caseload and level of care. Will present cases in staffing in a clear and concise manner * Help person(s)-served to develop or improve interpersonal, social, family, community and independent living skills.  * Follow up with all no shows by telephone or caring letter and will document attempts to re-engage within 72 hours of missed appointment  * Participate in discharge planning with the Psychiatrist and person served and those not participating in services  * Complete required documentation in a timely manner. * Adherence to all applicable evidence-based practice models.  * Orally communicate information effectively and accurately.  * Assure program operates within the constraints of the agency’s certification (CARF, DMH) standards and strategic plan related to clinical care.  * Abide by program and agency policies and procedures  * Other job duties and special projects as assigned.
