Opportunity Information Palo Alto Foundation Medical Group is seeking a full-time, BC/BC Palliative Care Physician. Resource-rich working environment. Palliative Care and Support Services (PCSS) is an innovative and nationally recognized leader in delivering integrated, multidisciplinary community-based palliative care. Our outpatient palliative care practice reaches patients where they are: clinic, home, assisted living, nursing facilities and via telemedicine. Collaborate with talented team of on-site primary care clinicians and medical specialists to provide comprehensive, coordinated and compassionate care for patients and families facing serious illness. Formulate comprehensive care plans, patient education and ongoing evaluations in collaboration with the Sunnyvale palliative care interdisciplinary team. Full-time is 35 patient contact hours. Weekly schedule is 5 days per week. Work/life balance with regular hours; no night shifts or weekend call. Average of 3-5 patients per day.
