Short Description:
Access Alliance Multicultural Health and Community Services (AAMHCS) is a Community Health Centre that aims to provide services and addresses system inequities to improve health outcomes for the most systemically vulnerable immigrants, refugees, people without status and their communities. The Centre envisions a future in which Toronto’s diverse communities achieve health with dignity.
AAMHCS is offering the opportunity for a physiotherapist to work in an interprofessional team including but not limited to medical doctors, nurse practitioners, nurses, counsellor therapists, health promoters and settlement workers. As a member of the team the candidate will be contributing to the care and support of individuals with diverse backgrounds and experiences so they may achieve and maintain good health and wellbeing.
The candidate who fills the position will have a global health experience and work in a setting with opportunities to expand and hone a range of clinical and health supportive skills, experiences and knowledge. Additionally, the candidate will enjoy an environment that supports strong clinical decision making and team-based collaboration, grounded in evidence-based practice.

Responsibilities
  • Provide individualized assessment, diagnosis, treatment, health education and counselling for acute and chronic musculoskeletal or neurological dysfunctions.
  • Work with members of interdisciplinary team (family physicians, nurse practitioners, dietitians, and social workers) to delivery comprehensive care.
  • Refer clients to appropriate internal and external resources.
  • Maintain complete and accurate electronic medical records.
  • Assess for assistive devices (mobility equipment) and complete applications, where appropriate, for the Assistive Devices Program.
  • Participate in chart reviews, case conferences, monthly meetings, and quality improvement initiatives.
  • Participate in the Physiotherapy Network of TCLHIN CHCs.
