JOB SUMMARY: The Physiotherapist/Professional Practice Leader in the Transitional Care Unit (TCU) plays a dual role in delivering high-quality, patient-centered rehabilitation services while providing clinical leadership to the physiotherapy team. This position is responsible for assessing, planning, and implementing individualized treatment programs that support patients – primarily older adults-through recovery and transition between care settings. The Professional Practice Leader also ensures clinical excellence by mentoring staff, promoting evidence-based practices, and contributing to program development and quality improvement initiatives. Working collaboratively within the interdisciplinary team, this role support safe discharge planning, enhances functional outcomes, and fosters a culture of compassionate, coordinated care. DUTIES AND RESPONSIBILITIES: Provide comprehensive physiotherapy assessments and individualized treatment plans for patients transitioning between care settings Deliver evidence-based interventions to support mobility, strength, balance, and functional independence Collaborate with interdisciplinary team members to develop and implement patient-centered care plans Educate patients and families on rehabilitation goals, exercised, and strategies to support recovery and prevent decline Support safe discharge planning by assessing equipment needs, home safety, and community resources Lead and mentor physiotherapy staff, promoting best practices and clinical excellence Facilitate professional development through training, coaching, and performance feedback Monitor and evaluate clinical outcomes, ensuring quality improvement and adherence to standards of care Participate in program development, policy review, and strategic planning initiatives withing the TCU Maintain accurate documentation and contribute to data collection for reporting and evaluation Collaborating with physiotherapists across the various TCU sites OTHER SKILLS AND ABILITIES: Strong knowledge of physiotherapy principles, geriatric rehabilitation, and transitional care practices Adhere to the policy and procedures for Bayshore Healthcare Ability to mentor, guide, and support clinical staff while promoting a culture of excellence and continuous improvement Skilled in working with interdisciplinary teams to develop and implement patient-centered care plans Excellent verbal and written communication skills for interacting with patients, families, and the interprofessional team Strong assessment and problem-solving abilities to address complex patient needs and care transitions Comfortable working in a dynamic environment with changing patient needs and system demands Familiarity with electronic health records and digital tools used in clinical documentation and care coordination Participate in ongoing internal and or external education activities Participate in proactive Health & Safety activities while performing all duties. Responsible to notify immediate Supervisor of a Health & Safety related issues. Ability to manage challenging and conflict using an objective approach Ability to work independently and to effectively collaborate with the interprofessional team Chart audits monthly to ensure adherence to College of Physiotherapy standards Commitment to continuous learning Other duties as assigned Master’s degree in Physiotherapy from an accredited institution Registered and in good standing with the College of Physiotherapists of Ontario CPR and First Aid Minimum 3-5 years of clinical experience, preferably in geriatric, rehabilitation, or transitional care setting Demonstrated leadership experience or interest in professional practice development Strong knowledge of physiotherapy assessment tools, treatment techniques, and discharge planning processes Commitment to evidence-based practice, quality improvement, and patient-centered care Experience working within interdisciplinary teams and supporting patients transitions across care sectors Valid driver’s license Current criminal reference check and vulnerable sector screen Bayshore is committed to a high quality and safe environment for both our staff and clients/patients.
