About the role The role of the Physiotherapist is to work within the Mission and Mission in Action of HammondCare to ensure a high standard of Physiotherapy service is provided to patients, along with appropriate support and advice to their carers. The permanent part-time (24 hours per week) Physiotherapist will participate as a member of the multi-disciplinary team in assessing patients’ level of functional mobility and care needs, setting care goals and providing appropriate treatment. The position will be based at the Northern Beaches Palliative Care Community Centre providing care for community Palliative and Supportive Care patients. You may be required to work across HammondCare North sites when required (Wahroonga, St Leonards and Macquarie). The successful candidate will participate as a member of the HammondCare team in the application of the organisation’s Mission and values to the pursuit of its objectives in a patient-centred, evidence-based, continuous quality improvement work environment that is consistent with the values and ethics of HammondCare and the Australian Physiotherapy Association. HammondCare is a NSW Health-affiliated organisation all NSW Health transferrable entitlements will be recognised. About you A tertiary qualification in Physiotherapy and registration as a Physiotherapist with AHPRA Experience in providing Physiotherapy Services in Outpatient Rehabilitation, or other Sub-acute areas of care (e.g., Palliative Care) Demonstrated effective communication and interpersonal skills (written and verbal) Demonstrated organisation and time management skills Demonstrated ability to work independently and as part of a multidisciplinary team Demonstrated commitment to professional development and ability to implement evidence-based practice.
