This is an exciting opportunity for a Physiotherapist looking for a varied role in Community Aged Care on a Full-Time or Part-Time permanent basis. This role involves assessing, planning and delivering individualised/group physiotherapy services and activities to improve the well-being of clients in their own homes as well as running group sessions across our Wellness Hubs. Make a difference in the fast-growing Community Aged Care space and work as part of a dynamic multi-disciplinary team to enable our clients to age well and maintain their independence, with a focus on what matters most to them.

Responsibilities

DAY TO DAY THIS ROLE LOOKS LIKE:

  • Conducting comprehensive assessments of our clients physical and functional abilities to gain an understanding of their needs
  • Developing and delivering individualised or group treatment plans based on clients’ needs, goals, and preferences
  • Implementing evidence-based physiotherapy interventions to address mobility issues, pain management, and other age-related conditions
  • Regularly assessing and reporting on the physical and psychosocial capabilities of clients to care management team
  • Maintaining, preparing and supporting precise records, exercise sheets, assessment reports and progress notes that accurately reflect clients’ physical function.
  • Making recommendations in relation to exercise programs, mobility equipment, exercise equipment, safety at home and referring to other disciplines as required.
  • Upholding ethical and professional standards in accordance with regulatory guidelines.
  • Collaborating with interdisciplinary teams, including care advisors, occupational therapists, fitness instructors, allied health assistants, and other healthcare professionals, to provide holistic care.
  • Educating clients and families on preventive measures and exercises to improve and maintain mobility.
  • Participating in team meetings, case conferences, and training sessions to contribute to the continuous improvement of care delivery
  • Engaging with team members, clients and client’s families in a positive and respectful manner, welcoming a culturally diverse team
  • Building relationships with clients and their families, understanding the challenges they face in the ever-changing landscape of aged care.

TO BE SUCCESSFUL IN THIS ROLE YOU’LL NEED:

  • Bachelor or Post Graduate Degree in Physiotherapy and current AHPRA registration
  • Excellent interpersonal, verbal, and written communication skills.
  • Ability to manage all aspects of compliance
  • Demonstrated experience with multidisciplinary team working.
  • Effective time management skills to ensure patient intervention, reports and outcomes are achieved in a timely manner and the ability to effectively prioritise and organise own work/caseload.
  • Demonstrated computer literacy
  • Demonstrated knowledge and experience of health, aged and/or disability services
  • Current Driver’s licence and registered and insured motor vehicle
