Manage a caseload of residents requiring rehabilitation support
Conduct comprehensive physiotherapy assessments and develop personalised treatment plans
Deliver evidence-based rehabilitation programmes focused on mobility, strength, and independence
Work alongside residents to establish meaningful and achievable rehabilitation goals
Support discharge planning and safe transitions back into the community
Collaborate with Occupational Therapists, Social Workers, Doctors, and wider healthcare teams
Provide guidance and supervision to therapy assistants
Participate in multidisciplinary meetings and case reviews
