Description:
To be responsible for own caseload following referral from a Physiotherapist , adhering to specified therapeutic programs and protocols, monitoring patient progress and adapting treatment as necessary. Supporting discharge planning for patients including completion of discharge paperwork. Supporting other support workers to manage their caseloads, support support worker training and working with undergraduate students out on placement
