Description PURPOSE: To secure safe, appropriate placements for all children in out-of-home care. KEY RESPONSIBILITIES: · Secures safe, appropriate placements for all children in out-of-home placements. · Documents all placement changes and provides information to Data manager and document in FSFN. · Attends all Placement Preservation staffings pertaining to placement activities and weekly SCC Shelter staffings. · Attends court when needed. · Responds to daily placement related emergencies and performs on-call responsibilities on a rotating basis. · Assists in the retention efforts of present foster/adoptive homes. · Maintains formal and informal contacts with other agencies and groups. · Maintains accurate and up-to-date information regarding resources and provides information and referral services based on case assessment. · Performs with appropriate supervision from supervisor. · Maintains accurate and up-to-date information about program policies and procedures for which employees are responsible and adheres to these appropriately during the course of job performance. · Assists in the training/licensing of foster/adoptive parents. · Participates in Foster/Adoptive parent recruitment activities. · Participate in the on-going development and related activities of a comprehensive quality improvement process within the Child and Family Services division. · Attend monthly residential meetings. · Focus on stepping children out of group care. Responsibilities
The Placement Specialist is responsible for securing safe and appropriate out-of-home placements for children and managing all related documentation. They also provide referral services, attend court hearings, and participate in recruitment and retention efforts for foster and adoptive homes.
