APH offers customized services for Medicaid, Medicare, and the ACA/Marketplace lines of business.       POSITION PURPOSE     The Population Outcomes Management (POM) Operations Manager plays a vital role in ensuring effective coordination, oversight, and optimization of programs designed to improve patient outcomes in population health management. This role supports the strategic execution of initiatives related to Population Outcome Management.  The POM Operations Manager will be responsible for overseeing day-to-day operational functions, aligning cross-functional teams, and ensuring adherence to established performance metrics, regulatory requirements, and best practices. This individual will serve as a bridge between clinical, operational, and technology teams, ensuring that patient-centered care is delivered efficiently and consistently, while driving measurable improvements in population health outcomes.  This position works collaboratively with clinical leadership, quality teams, IT, and external stakeholders to strengthen population health initiatives.        DUTIES & RESPONSIBILITIES    Program Operations & Oversight  * Oversee the daily operations of POM programs, ensuring that patient engagement, care coordination, and follow-up processes are effectively implemented. * Monitor operational workflows for population-based and respiratory care programs to ensure compliance with clinical protocols and organizational goals. * Support the design, implementation, and ongoing refinement of outcome-based interventions for patients with chronic and complex respiratory conditions. Performance Monitoring & Reporting  * Track, analyze, and report on key performance indicators (KPIs) tied to population outcomes, patient satisfaction, readmission rates, and clinical improvement measures. * Develop dashboards and regular operational reports to present to senior leadership, highlighting program effectiveness and opportunities for improvement. * Ensure alignment with payer and regulatory reporting requirements. Quality & Compliance  * Partner with Quality and Compliance leadership to maintain adherence to evidence-based practices, accreditation standards, and risk mitigation strategies. * Lead internal reviews and audits to ensure programs meet quality, compliance, and outcome objectives. * Support quality improvement initiatives focused on reducing care variation and optimizing resource utilization. Team Leadership & Collaboration  * Manage a team of coordinators, analysts, or support staff within POM operations, providing coaching, mentorship, and ongoing development. * Collaborate with clinical providers, IT teams, case managers, and external partners to streamline patient care pathways and ensure effective communication. * Cultivate a culture of accountability, innovation, and patient-centered excellence. Process Improvement & Innovation  * Identify operational inefficiencies and implement process improvement initiatives to enhance care delivery and patient engagement. * Contribute to strategic planning and resource allocation for new and existing POM initiatives.
