Its goal is to support our sponsors and partners in their strategic evolution to positively impact and improve the healthcare delivery system. Make a Difference The Population Health Advisor, IHCI, has responsibility for driving value-based care initiatives in Deaconess’s regional practices near and around Jasper, Indiana. This position works closely with the Quality and Analytics teams and helps integrate clinical programs, network saving initiatives, quality initiatives, and resources into the practice workflows with the intent to optimize the patient experience and positively impact physician engagement and quality of care provided. Exceptional Skills and Qualifications Applicants for this position should be able to communicate effectively with internal team members, and key stakeholders – including public speaking, PowerPoint, dashboards, visualizations, and/or other presentation formats. Applicants must be highly organized and self-motivated individuals with the ability to adapt to various workspaces and work autonomously. This applicant has a collaborative work style with the ability to work across different teams, areas of expertise, and adapt to ambiguous environments and clientele. Bachelor’s degree or equivalent work experience required. 5 or more years of leadership experience in practice management. Provider relations, and project management. 3 or more years of experience in practice re-design work, including Patient Centered Medical Home, Practice Transformation, Quality Improvement, ACO development, Ambulatory Care, Quality and Efficiency metrics, and Electronic Medical Records. Health plan experience with a provider perspective and strong orientation to value-based care principles and the health policy landscape. Experience in data analytics comprehension. Must have a valid Driver’s license and the ability to travel regionally up to 50%25 of the work week. Proficient in Microsoft Office products (Excel and PowerPoint). Hypothesis-driven to identify trends, predict issues, highlight critical areas, and develop corrective action plans.
