ESSENTIAL FUNCTIONS:

• Develop and implement population health strategies that align with organizational goals and enhance patient care,
• Oversee clinical programs focused on disease management, preventive care, and health promotion for diverse populations,
• Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and community partners, to create integrated care plans
• Utilize data analytics to identify trends in health outcomes, patient engagement, and resource utilization, and implement strategies for improvement
• Monitor and report on key performance indicators related to population health initiatives,
• Ensure compliance with regulatory and quality standards in all population health activities,
• Conduct training and education sessions for clinical staff on best practices in population health management,
• Facilitate community outreach efforts to engage and educate patients on available resources and health initiatives,
• Act as a liaison between clinical teams and leadership to advocate for patient-centered care practices and policies,
