Description
The peer will utilize lived experience to model recovery and support patients who are struggling with mental health and/or substance use disorders. They work directly with patients in the ED, inpatient, and outpatient settings and support a person-centered approach to care. This includes identifying appropriate resources and facilitating linkage to care. The post holder collaborates with the patient’s support system and interdisciplinary treatment team to encourage engagement in services and reduce barriers to recovery. * Manages designated caseload of members who could benefit from peer services, care coordination, and health navigation * Visits patients at the bedside and in clinic settings. * Schedules and conducts multiple field-based visits per day. * Completes assessments and questionnaires to identify barriers and opportunities for intervention * Works telephonically to make outreach calls and receive incoming calls to/from members regarding care coordination. * Provides referrals for outpatient services and community-based resources, to assist with achieving goals, caring for medical and behavioral health concerns and support social care needs. * Collaborate with service providers such as doctors, social workers, discharge planners, and community-based providers to ensure care is person centered, coordinated and needs are addressed * Communicates with patients through telephone, mail, MyChart and text messages where appropriate, to increase patient attendance. * Documents and maintains patient-specific records. * Maintains accurate daily logs for statistical reporting purposes and for reviews of productivity. * Meets department leaders and attends team meetings to discuss progress and the work of the team. * Checks and responds to E-Mail and teams messages throughout the day. Qualifications A. Education Required - High school diploma / GED B. License/Certification/Registration(s) Required - Certified Recovery Peer Advocate (CRPA) C. Experience/Skills Required:      • Proficiency in English and Spanish, preferred      • Demonstrated ability to manage a caseload and work effectively within a team, in a fast-paced environment      • Work experience requiring effective communication and engagement skills, as well as healthy boundary-setting      • Proficient computer skills, including a solid understanding of Microsoft Outlook, Word and Excel
