Description: Under pharmacist supervision, supports the pharmacy department and medical practices in ensuring adherence to medications and working with payer partners to help with pharmacy related metrics. Coordinates with pharmacists and clinical team members system-wide, pharmacies as well as external healthcare partners. Key job duties may consist of patient chart reviews, telephonic outreach and communication, medical record documentation, and data collection and management. Works with patients, pharmacies and clinical teams to resolve barriers to medication access.
