Community Health Social Work North is a dynamic, values-driven team delivering evidence-based, trauma-informed Social Work services across community health centres, district hospitals, and rural and regional communities throughout Northern Tasmania. Our clinicians work with people experiencing a broad range of health and psychosocial challenges. This requires strong therapeutic counselling skills, sound psychosocial assessment, and the ability to support people through complex issues such as grief and loss, adjustment to illness, trauma, stress, mental health concerns, family and relationship pressures, social isolation, carer stress, and life transitions. From Launceston to surrounding rural communities, the role offers meaningful variety and the opportunity to build strong therapeutic relationships over time. This is a role suited to experienced Social Workers who are confident working with autonomy. THE ROLE In this role, you will provide Social Work services to individuals, families, groups, and communities within the Launceston area, plus have the opportunity to provide outreach to surrounding regions and district hospitals. Travel during work hours is fully supported, with access to a fleet vehicle. No two days are the same. Your work may include comprehensive psychosocial assessment, therapeutic counselling, medium to long-term intervention, group facilitation, outreach visits, care coordination, advocacy, community development, and integrated multidisciplinary planning. You will work with people experiencing a broad range of health, mental health, social, emotional, relational, and practical stressors. This may include supporting people adjusting to illness or life changes, navigating complex systems, managing grief and loss, trauma, stress, family or relationship concerns, social isolation, carer stress, or challenges linked to rurality and limited-service access. This role requires an experienced clinician who can work confidently with minimal day-to-day supervision, while actively engaging in reflective practice, professional supervision, and team-based collaboration. YOU WILL NEED You will be an experienced Social Worker with demonstrated skills in: * Providing therapeutic counselling, medium to long-term 1:1 Social Work intervention, comprehensive psychosocial assessment, formulation, care planning, and risk-informed practice across a broad range of health-related, psychosocial, emotional, relational, and practical issues. * Working autonomously with sound clinical judgement in varied community-based, outreach, sole-practitioner, rural and regional settings, while managing complexity, prioritising competing demands, and remaining connected to supervision, peer support, and a broader professional team. * Building trusted relationships with clients, local communities, multidisciplinary teams, health services, government and non-government agencies, including through advocacy, consultation, system navigation, collaborative care planning, and the design and facilitation of therapeutic or psychoeducational group programs. DETAILS OF APPOINTMENT Multiple fixed term, part time, day worker positions, working up to 76 hours per fortnight, commencing as soon as you can join us for a period of 6 months. *Notwithstanding hours to be negotiated with the successful applicant. SALARY $117,272 to $126,182 pro rata per annum. Our Employer 12% superannuation contribution is on top of this amount. * Salary range is in accordance with Allied Health Professionals Public Sector Unions Wages Agreement 2025. SALARY PACKAGING  You’ll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.
