This advanced practice role provides clinical leadership, patient education, and care coordination for patients living with Chronic Kidney Disease across the continuum of renal care. Working collaboratively within a multidisciplinary renal team, the CNC – CKD plays a key role in supporting patients and families through education, dialysis preparation, vascular access coordination, and ongoing management of chronic kidney disease. The successful applicant will utilise advanced clinical knowledge and leadership skills to improve patient outcomes, support evidence-based practice, and contribute to quality improvement initiatives within a specialised renal service. THE ROLE: KEY RESPONSIBILITIES * Deliver nurse-led education and support for CKD patients and families * Coordinate pathways for dialysis access and renal replacement therapies * Provide clinical leadership and expert renal nursing advice * Support continuity of care across haemodialysis, peritoneal dialysis, and transplantation pathways * Contribute to quality improvement, research, and evidence-based practice initiatives * Work collaboratively within a multidisciplinary healthcare team ABOUT YOU The successful applicant will demonstrate: * Significant renal nursing experience and advanced clinical knowledge * Strong communication and interpersonal skills * Ability to work autonomously and collaboratively within a multidisciplinary environment * Commitment to patient-centred care and continuous quality improvement * Experience in education, care coordination, and change management within healthcare settings Note: - Please refer to the Statement of Duties for all the selection criteria of this position to address in your application. DETAILS OF APPOINTMENT Permanent part time, day worker (with on call) position, working up to 38 hours per fortnight, commencing from as soon as you can join us. *notwithstanding hours to be negotiated with the successful applicant. SALARY $118,297 to $125,543 per annum (Pro rata).
