The Team: CYMHS are seeking new H2HK Tasmania Clinical Nurse Specialists Practitioners. This role will work as part of a multidisciplinary team, inclusive of a Clinical Lead, Team Leader and Medical Officers, to deliver secondary level mental health assessment and interventions for infants, children (0-12), and their families and caregivers, experiencing emerging complexity regarding emotional, social, relational, and behavioural functioning. This early intervention service enables families to access a range of evidence based individual and group interventions and supports for mild to moderate emotional, behavioural, social, relational, and developmental challenges. The Head to Health Kids Tas service works with families, and universal, secondary, and tertiary service providers, to provide integrated and holistic care. The Department for Education, Children and Young People (DECYP) is a key co-service partner. The Role: * As part of the Head to Health Kids team, support the establishment of the Northwest service * Undertake assessments, care planning and delivery of individual and group secondary level mental health interventions to children and families, including culturally appropriate services. * Develop and support collaborative care partnerships with service providers including across primary health, child health and parenting, paediatrics, child safety, education, and early childhood programs and services. * Collect and review client and service outcome data and participate in activities to improve client outcomes. * Contribute to continuing quality and safety, service development and improvement, and professional development processes and standards. You will need: * A minimum of two years post graduate employment in a clinical area directly applicable to the Child and Adolescent, Adult Community, Inpatient and Extended Treatment and Older Persons Mental Health Service area. * Demonstrated clinical knowledge and understanding of mental health in relation to the Child and Adolescent, Adult Community, Inpatient and Extended Treatment and Older Persons Mental Health Service area with the ability to provide appropriate assessment of mental health problems. * Demonstrated expertise in the theoretical and practical application of a range of therapeutic interventions relevant to the Child and Adolescent, Adult Community and Older Persons Mental Health Service area with the ability to acquire further expertise into the future.
