[https://publicstorage.dc2.pageuppeople.com/384/ProviderAsset/scribble/c0fcfc01-a3db-4665-a615-fc173e9e865f.png]THE TEAM * Working within the dynamic Hospital Aged Care Liaison Team (HALT), collaboratively with the Residential Aged Care Home Liaisons (RACHLs) and the Transition Care Program (TCP) teams to support safe, coordinated, and person‑centred transitions for older persons across hospital, residential aged care, and community settings. This integrated approach ensures timely communication, comprehensive assessment, and continuity of care for individuals with complex needs. THE ROLE * Provide expert clinical advice and leadership within an interdisciplinary team providing assessment, care coordination and implementation of planned outreach support to older people in the emergency department, community or residential aged care setting to enable care needs to be met in the most appropriate environment. * Undertake a broad consultative role by initiating and maintaining a comprehensive care network, building and maintaining links with providers in the primary health setting, including community service providers, General Practitioners, Residential Aged Care Facilities and the Rural Inpatient Facilities. * Develop and implement evidence-based practices that promote the efficient and effective provision of patient centred aged care services within the healthcare setting. YOU WILL NEED: * Knowledge working with the older persons within the acute care and community settings. * Experience in a Clinical Nurse Consultant role working with the older persons. Note: - Please refer to the Statement of Duties for all the selection criteria of this position to address in your application. DETAILS OF APPOINTMENT * Permanent, full time, shift worker position, working up to 76 hours per fortnight, commencing from 25 October 2026. *notwithstanding hours to be negotiated with the successful applicant. Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies. SALARY $118,297 to $125,543 per annum. Our Employer 12% superannuation contribution is on top of this amount. * Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023. In addition, the following allowances calculated on the salaried incremental point may be available: * Post Graduate Allowance of up to 10% based on qualification type SALARY PACKAGING You’ll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.
