Are you a professional nurse with experience in Post Acute or Acute Care patient interaction? Are you looking for an employer that will appreciate and reward your hard work? If so, this job opportunity might be for you! We are seeking to add a Post-Acute Liaison to our team!
A full-time Post-Acute Liaison’s primary responsibilities include clinical screening and assessment of the potential patient/resident, assisting in the facility admission process and ensuring a smooth transition of the patient to the next level of care. While managing patient placements from the hospital setting to one of our Skilled Nursing & Therapy Centers, they will also work to partner/network with other health care professionals.
QUALIFICATIONS
- Must be a licensed nurse (RN or LPN)
- Experience as a hospital liaison preferred
- Experienced in Skilled Nursing or Skilled Rehabilitation environment
- Clinical Sales or Business Development experience
- Reliable transportation required
Responsibilities:
- Develop strong business relationships with key hospital referral personnel, including Case Managers and physicians.
- Networking and business development with hospitals, LTACs and physicians groups to provide a consistent source of transitioning patient referrals.
- Perform pre-admission patient screening/assessment, including clinical, equipment and transportation needs.
- Work in a collaborative manor with the facility Admissions Coordinator to create an exceptional patient admission experience.
- Serve as an educational resource for patients, families, hospitals and healthcare professionals
- Responsible for administrative and procedural requirements relating to territory accounts; including recording daily activity within the referral system.
