JOB DESCRIPTION

The Practice Nurse is responsible in the first instance to the Lead Practice Nurse and thereafter to the Practice Manager & Partners.

DESIRED SKILLS

To have undertaken training in Diabetes Management with an accredited trainer to manage patients with Diabetes. To run specific clinic sessions both 1:1 and as a group consultation with the support of a facilitator. Be willing to attend weekly Diabetes Team meetings with the Diabetes Lead GP and Community Diabetes Nurses to discuss caseloads and management of specific patients. To proactively support the practice in running robust recall systems to ensure patients are receiving essential monitoring to support them to reduce disease progression. Training would be considered for the right candidate.

ESSENTIAL SKILLS

  • New patient health checks and NHS health checks
  • Venepuncture
  • Urine testing/analysis
  • Blood pressure measurement/monitoring

The Practice Nurse may, from time to time, be asked to undertake other reasonable duties, with appropriate training. It is important that all members of staff are prepared to undertake additional or relinquish current duties to maintain service delivery to patients. It is expected that requests from the Partners/Management Team/Lead Practice Nurse will not be unreasonably refused.
