JOB SUMMARY:
Work within Nursing Team delivering a high standard of care. Liaising with all care disciplines
Provide emergency care to patients when required
Will provide general and specific health screening to the practice population (within agreed protocols) as per GMS practice. Supporting Nurse Manager to achieve QOF targets.
Will maintain professional development by attending relevant courses and study days as agreed by line manager.

· These tasks will be introduced to the role over time as appropriate. Training will be untaken to support the practitioner in the tasks. All tasks should be completed using Practice policies and protocols.
· Will assess patient’s needs and follow agreed protocols.
· Phlebotomy
· CHD Clinic / Asthma Clinic / COPD Clinic / Diabetes Clinic / RA / Learning Disabilities / Mental Health / PVD / Hypertension / CKD / Dementia - Will see patients for annual reviews as required following agreed protocols and refer to GP.
· Injections – (Including B12, Prostap and season vaccinations) will be given as required following in-house and NHS England/ ICB requirements.
· Travel Advice and Vaccinations – This will include any advice that would be required to ensure patient safety, i.e.; travel.
· Dressings – Will be applied as per wound care formulary and the relevant paperwork completed for those required external referrals local protocols shall be followed.
· Removal of Sutures.
· Chaperone duties
· Child & Adult immunisations as per NHS England protocols.
· Organisation of waste management, including weighing & preparing for collection
· Smears / swabs – Practitioners will be suitably trained and follow local guidelines.
